POSITION SUMMARY:
Under the direction of the Board of Health, Director of Public Health, Outreach & Prevention Coordinator, and/or their designees, the Outreach Clinician/Case Manager will work as part of the Street Outreach Team to provide assessment, referral, and coordination services for unhoused individuals in Fitchburg experiencing substance use disorders and/or mental health challenges. This position will serve as an integral member of a progressive public health team—including
the Health Director, Street Outreach Team, and Fitchburg Police Department’s Community Engagement Unit—providing outreach and services in a variety of settings. The Clinician will also assist the Fitchburg Police Department with follow-up outreach and visits to individuals with co-occurring disorders and will be based at the Fitchburg Health Department Street Outreach location.
RESPONSIBILITIES AND DUTIES:
Duties include but are not limited to:
• Provide assessment, referral, and coordination of care for unhoused individuals experiencing substance use disorders and/or mental health concerns identified by the team;
• Coordinate treatment services with providers, including detoxification, outpatient care, residential placement, and other appropriate services;
• Assist the Street Outreach Team, Police Department, and families with Section 12 and Section 35 assessments;
• Attend regular Street Outreach and Fitchburg Police Community Engagement Unit meetings;
• Participate in monthly HUB meetings;
• Support community health education, outreach, and awareness efforts as appropriate;
• Maintain collaborative relationships with local mental health and substance use treatment agencies;
QUALIFICATIONS, SKILLS and ABILITIES:
• Master’s degree in a clinical behavioral health field (mental health, psychology, counseling, or related discipline);
• Must possess active Massachusetts licensure as an LMHC;
• Minimum of three (3) years of experience in a community-based setting, including clinical evaluations and outreach to underserved populations;
• Familiarity with Section 12 and Section 35 processes, court practices, and applicable federal, state, and local laws and regulations;
• Experience working with unsheltered individuals and those with substance use and/or co-occurring disorders;
• Familiarity with local programs, resources, and organizations serving individuals with behavioral health needs;
• Proficient with Microsoft Office products, including but not limited to Word and Excel;
• Ability to collect and maintain data and documentation as required;
• Possess a valid Massachusetts driver’s license and a reliable vehicle (travel expenses reimbursed by the City of Fitchburg);
• Responsible for maintaining assigned City-issued equipment (cell phone, tablet, etc.);
• Position may include evening and weekend hours;
• Must be honest, dependable, and maintain a high level of integrity;
• Familiarity with the City of Fitchburg and local community organizations preferred;
Salary Range:
$40.34/hr. – $43.22/hr. Commensurate w/Qualifications and Experience
Hours: up to 15 Hours per week – May include some nights and weekends as required.
